We received the following information today from the TMA that applies to providers in all states.

Blue Cross and Blue Shield (BCBS) Settlement Update

The BCBS settlement claim forms are being mailed to physicians on July 27.

Prospective class members can expect to receive a plain white envelope with a return address referencing the settlement administrator:

Blue Parties' Settlement Administrator
PO Box 2876
Portland, Oregon.

The documents which should be enclosed are:

• Blue Cross Notice
• Claims Form
• Claim Instructions

Please note that Section D of the Claims Form should reference page 4 (not 3) for the list of eligible organizations.

In order to qualify for a settlement payment, physicians must mail the completed and signed Claim Form by NO LATER THAN OCTOBER 19, 2007.
